*HOME CARE SERVICE**

_Patient full medical history and diagnose_

_Head to toe full examination_

_Any abnormalities like skin discolouration, bleeding from post operation, pressure sore, bruse, strength in arms & legs, mobilities, nutrition, medications, hidration, urine output, fluid intake, bowel movements, communication levels, on nappies or catheter, Doctor and Hospital if need to be transferred to hospital, medical aid and plan on_

*NURSING CARE PLAN*

*Daily Exercise*

_Physiotherapy, Speech therapy, Occupational therapy. If patient is under a therapist or else his/her daily routine exercise will be given_

*Hygiene*

_Assist in showering or bathing or full bed wash. Male patient will be shaved if can shave himself. Depending on patient condition and diagnose_

*Catheter Care*

_Will be done for patient that does have catheter in, whether Urinal Catheter or Urinal Catheterisation_

*Wound Care*

_Wound care and Wound dressing done if patient have own supplies or nurse will supply wound dressing_

*Nutrition*

_Assist in feeding if needed,tube feeding if patient have a feeding tube in or else make sure patient eat a proper diet_

*Medications*

_Will be given as prescribe by Doctor and noted if patient takes own medication_

*Vital Signs*

_Blood Pressure, Pulse, Respiration, Temperature, SPO², HGT (sugar once for none diabetic and regularly for diabetics), Urine Test (once a week)

*Transfer to Hospital*

_Nurse remains with patient when transferred from home to hospital via ambulance. Remain with patient in casualties to handover to hospital staff and Emergency Doctor. Remain in communication with family to inform which procedure or tests are going to be done and if admitted to ward. Remain with patient when admitted to ward handover to ward Sister. Will check up on patient on regular base day/night

*Palliative Care (Quality of life or End of life)*

_Families will be taking through the whole process and with everything with reason why certain things are done and what to expect to see. If patient past away family don't have to worry as what to do next. Nurse will do the rest like getting police document, ambulance death clearance and getting a undertaker to take the body to the morgue. If patient doesn't have a funeral policy, Nurse does have a trustworthy Funeral Service to take care of your loved one. Death certificates will also be done and if request a autopsy can also be arranged for family_
